Getting Started
Fill out the intake packet
Prior to meeting in person, you should fill out the appropriate intake packet. We will review the information in this packet in detail during our first therapy session. Please click on the appropriate link below to open and/or save the form. You should print out this form, fill it out and bring it with you to our first therapy session:
- Minor Intake Packet (For children and adolescents under age of 18)
- Adult Intake Packet (For adults over the age of 18)
If you would like me to coordinate care with another professional (e.g., your psychiatrist, primary care physician, teacher, etc.), please complete the applicable release of information form below:
Meet in person
The first session is considered the intake session. At this time, we will review the intake packet which you should fill out prior to the session. You will have an opportunity to get to know me and I will get a better understanding of your concerns. At the end of the first session we will decide on a regular time to meet on a weekly or twice a week basis.
